Jacksonville Pediatrics is pleased to have you as patient. We accept most major insurance plans for your convenience. PLEASE REMEMBER: Your insurance is an agreement between you and your insurance company. We file your insurance as a courtesy to you but it is ultimately your responsibility to know what your insurance benefits are, what is covered and not covered, and what you are financially responsible for. Insurance has become very complicated and even within the same insurance company there may be many different plans. Depending on the options your employer chose your plans can very greatly!
Special Notices:
As of September 2007, Jacksonville Pediatrics no longer accepts personal checks at the office. We take debit cards, and most major credit cards.
Jacksonville Pediatrics charges a $25 no show fee for appointments not cancelled 24 hours in advance.
Please come to appointments on time. Patients more than 15 minutes late may be asked to reschedule.
Attention: Divorced and unmarried parents:
It is the policy of this office that the parent accompanying the child for treatment will be held responsible for all copayments/deductible payments etc at the time of the service regardless of which parent has primary custody of the child or which parent holds the insurance policy on the child. Again, the parent who brings the child in must be prepared to make all required payments at the time of the service.
